Health Disparities Collaborative

PHC, as leaders in managing quality improvement, and moving toward the elimination of health disparities in our community, has implemented key elements to improve health outcomes among patients with chronic diseases. Currently, PHC has developed a chronic disease management program. The program addresses patients diagnosed with diabetes. Asthma and other chronic illnesses will be addressed in the near future.

Minority Health Program

The Minority Health Program serves minority adults (19 year and older). Along with the medical services received, cardiovascular disease is one major illness monitored among enrollees. Also, patients in the program receive health education counseling, medication management and other medical information that discusses measures to improve their health.

Prenatal Care Program

PHC’s goal is to provide prenatal care to women in their first trimester. Women who seek early entry to prenatal care reduce the rate of infant mortality. Having healthier moms creates healthier babies. This program’s services are provided by Certified Nurse Midwives. Our nurse midwives deliver our newborns and follow-up post-partum care is received at PHC.

Medication Assistance Program

PHC offers several prescription assistance options to its uninsured and underinsured patients. Through various pharmaceutical companies, PHC staff complete applications for patients to obtain their medication at an affordable cost.
